The following accident reports show common causes of accidents related to lifeboat equipment. These accidents are not attributable to non-OEM maintenance and would not have been prevented by MSC 1206.

Equipment in a satisfactory condition, thereby implying that accident was due to operator error. Second report identifying corrosion of link plates or keel shoe. Should be spotted by PSC or RO surveyors.
